Do you accept my health insurance?

Some of our doctors are credentialed with numerous insurance providers, including but not limited to: Blue Cross/Blue Shield Anthem, Humana, Cigna, Aetna, Medicare, KY Medicaid, Caresource, United/Optum and Tricare. Questions about coverage should be directed to your doctor.

What is your private rate?

Each doctor’s private fee is different. Please contact your doctor to discuss his or her rates.

If I am using health insurance is pre-authorization needed?

It depends. Nowadays fewer health insurance carriers require preauthorization for outpatient mental health. However, you should never assume. Contact your health insurance provider and inquire.

What is my copay for a mental health session?

Copays vary by insurance plan. Often times it is noted on your health insurance card. We recommend calling your health insurance provider to determine all benefits before starting services.

Do I have a deductible, and if so has it been met?

This also varies by insurance plan. Again, we recommend calling your health insurance provider to determine all benefits before starting services.

What forms of payment do you accept?

Payment should be made out and directed to your individual doctor and not to the Group. Currently, Dr. Bloch accepts only cash and check payments. Dr. Purvis accepts cash, check and some digital methods. Dr. Blaisdell accepts cash, check and debit/credit/HSA payments which can be made through the Payment Section on the website. Dr. Katz also accepts cash, check and debit/credit/HSA payments which can be made through the Payment Section on the website.
